Online Sign Up Form
(1) Online Sign Up Form does not include digital payment, all payments have to be done at the booth in SMU.
(2) Online Sign Up Form is designed for 1 member’s particulars, 1 team has to fill up the form 4 times in total.

::Booth Registration::

Venue: SMU Library Level 1 (Outside TCC)
Date: 11 & 19 Feb                    3pm – 7pm
            20, 26 & 27 Feb           12pm – 6pm
            5 & 12 Mar                     3pm – 7pm
            6, 7 & 14 Mar                12pm – 6pm

::Booth Registration Instructions::

(1) Payment by CASH only.
(2) Payment is on first-come-first-serve basis.
(3) Teams are accepted into race upon full payment only.
All participants are required to bring/submit:
     (4) a photocopy of swimming proficiency certification & kayaking proficiency
            certification (Open Category) for submission during payment at booth.
     (5) the original copy of certifications for verification.
     (6) submit a copy of Indemnity Form (below).
(7) The organiser reserved all rights to reject any registration if any participant(s) is/are unable to fulfill the procedures  as stated above.

::Water Proficiency Test::

You can swim but NO water proficiency certification?
We have 3 water proficiency tests for you.

Test Dates: 2nd, 4th & 10th March 2010
Time: 4pm
Venue: SMU Administration Building

All SMU School Challenge teams falls under Novice Category.
