Terms and Conditions for Loaning and Returning of Stores
The logistic holdings of SMUX are being placed under the jurisdiction of the Logistic Manager and the Logistic Committee. All team, event or individual request for logistic support needs to be requested and approved through proper procedures. Terms and conditions are to be followed completely.
a. A single individual should be appointed as the one responsible for all logistic operations with the SMUX’s logistic committee. He or she is often but needs not be the Logistic IC of the event. He or she is referred to as the “Lessee”.
b. The Lessee, upon accepting of receiving of the stores by signing on the Loan Document, acknowledges the received items to be in good functional conditions or as recorded.
c. The Lessee would be responsible for the well-beings of the stores before, during and after the event to the point of the logistic committee’s (Lessor) acknowledgement of return.
d. The Lessee is to ensure prompt return of the stores as stated on the loan document.
e. The Lessee is to ensure the designated representative from the logistic committee acknowledges the return of stores in goof functional condition by signing on the return document.
f. The Lessee would be responsible for recovering of the equipment to functional condition. In situations where replacement, the logistic committee would be responsible for doing so and payment would be incurred by the Lessee.
g. The Lessor retains title to the equipment
